Clinic Services 

The Community Clinic of High Point provides basic healthcare for adults of High Point who cannot afford health insurance and do not qualify for Medicare or Medicaid. At the Clinic we are able to provide acute care, like :

  • Medication maintenance 

  • Disease management

  • EKGs

  • Referrals 

  • Blood work lab services

  • On-site speciality office visits

    • Rheumatology​

    • Cardiology

    • Pulmonology

    • Neurology

    • Podiatry

Office visits are by appointment only. There are no walk in appointments.

CCHP No Show Policy

If an appointment must be cancelled, patients are required to call and notify CCHP 24 hours in advance of the scheduled appointment in order to not be charged a No Show fee.

If the patient is more than 15 minutes late and has not called to alert the Clinic, the patient will be counted as a No Show and will not be seen.

Clinic Visit Appointments

$10 fee for First No Show

$20 fee for Second No Show

Third No Show in a calendar year will result in a six month suspension from Clinic Services.

Eligibility Appointments 

$10 fee for First Re-Certification No Show

$20 fee for Second Re-Certification No Show

Third Re-Certification No Show in a calendar year will result in a six month suspension from Clinic Services.

Clinic Eligibility 

In order to be a patient at the Community Clinic off High Point you must be:
  • An adult, aged 18 to 64
  • A resident of High Point, Jamestown, Archdale, or Trinity
  • Have no private or government insurance (e.g Medicare, Medicaid, or VA Health Care benefits)
  • Live within the 300% of the Federal Poverty Level
If you do qualify, you will go through the eligibility process to verify that you do meet the Community Clinics requirements. You will have to bring the following documentation to your eligibility appointment.
  • Current photo I.D (with current address)
  • Social Security Card
  • Most current food stamp award letter or card
  • Most current 1040 tax return
  • If you are homeless, or someone is providing room and board for you, a letter of support is needed.
  • If you have a green card or visa, please bring it with you
  • Most current full month of pay stubs for all in household.
  • If you receive unemployment benefits, you will need to bring the most current award letter.
  • If you receive social security benefits or disability benefits, the award letter for the current year is needed.
  • We do request a $5 cash donation at each visit if possible.
