The mission of Community Clinic of High Point is to provide quality acute care and chronic disease management for eligible adults, aged 18-64, who reside in Archdale, High Point, Jamestown, or Trinity and do not qualify for any form of government or private insurance.

MEDICAL CARE SERVICES

Patients must have an appointment. No walk-in appointments are available.

Appointments are available:

Monday – Wednesday; 8:30am-12pm & 1:30pm-5pm 

Thursday; 8:30am-12pm

Prescription refill clinics are held Wednesdays from 1:30pm-5pm.

To avoid a No Show Fee, appointments must be cancelled 24-hours in advance.

If the patient is more than 15 minutes late and has not called to alert CCHP, the patient will be counted as a No Show and will not be seen.

Clinic Appointments

1st No Show: $10 fee

2nd No Show: $20 fee

3rd No Show: six month suspension from Clinic Services.

To qualify as a Community Clinic of High Point patient, you must:
  • Be an adult, age 18 to 64
  • Reside in High Point, Jamestown, Archdale, or Trinity.
  • Have no private or government insurance (e.g. Medicare, Medicaid, or VA Healthcare benefits.)
  • Live within 300% of the Federal Poverty Level.
 
After initial qualification, you will have an eligibility appointment. You will need to bring the following:
  • Current photo ID, with current address.
  • Social Security Card
  • Most recent food stamp award letter or card.
  • Most recent year’s 1040 Tax Return.
  • A letter of support if you are homeless or living with someone providing room and board.
  • Green Card or Visa, if applicable.
  • Household pay stubs for the most recent month.
  • Unemployment Benefit award letter.
  • Social Security or Disability Benefit award letter for the current year.
  • $5 cash donation, if possible.
